The practice has a catchment area of 5 miles. You will be registered with the practice and not with a specific GP, although you may name a preferred doctor and/or nurse and we will record this preference. You will be asked to complete 3 forms:

A New Patient Questionnaire Adults

A New Patient Questionnaire (Under 18yrs) (XLSX, 40KB)

Baby questionnaire only to be completed for newborn babies

Registration forms:

HS200 - to be completed by anyone joining the practice from another practice within Northern Ireland

HSCR1 - to be completed by anyone joining the practice from outside Northern Ireland

You will also need to provide:

• Photographic ID
• Proof of Address
• BSO may also request further information
• Confirmation from your previous GP of any medication you are currently being prescribed

You will be offered a New Patient Medical.

If you move or change any of your personal details (e.g. address, surname, phone number) please help us by informing us so that your records can be updated. 

Failure to do so could result in delays or prevent us from contacting you or a relative in an emergency.

Our facilities are located on the ground and are fully accessible for those patients using a wheelchair or any other mobility aids.
